All children participating in the Cavalry FC Regional Field House programming
will be asked to complete this
5-question digital COVID-19 health screening no more than 12 hours prior
to your arrival.
This tool will help you determine if it is safe for your child to enter the building. The screening data is secure and won't be collected or shared in any way - it's simply a way for you to know if it is safe for you and others to participate.
After completing this form, should your child's health status change, please refrain from entering the building.
